Application Instructions

PURPOSE
Recognizes a diabetes care and education specialist who has less than five (5) years of diabetes care and education experience and has demonstrated leadership and commitment in their practice.
AWARD
Recipient will be recognized at the ADCES Annual Meeting with a plaque, will receive a complimentary registration to the Annual Meeting, one-year Active Membership and a $1,000 travel stipend.
ELIGIBILTY
Applicant must be an active member of ADCES, as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award with less than five (5) years of diabetes care and education experience and has demonstrated leadership and commitment in his/her practice.
CRITERIA
Applicants for the Rising Star Award may be nominated by a peer/colleague or by self-nomination. Applicants must be an ADCES member who is in active practice of diabetes self-management education and support (DSMES) with no conflict of interest. Applicant should be an early careerist, with less than five (5) years in the field of diabetes care and education.

Letters of recommendation must include how the applicant provided the following:
  • Innovation and use of new approaches to the delivery of DSMES (description of the role the applicant played and the resulting positive outcomes that have occurred using innovative processes/approaches)
  • Working creatively in applying established and new scientific knowledge in DSMES
    • Fosters community connections (JDRF, ADA, AHA, YMCA, etc.). Evidence that the contributions the applicant has made to community connections are sustainable and have a positive impact on the community and/or the profession
    Current members of the Board of Directors and the Awards Committee are not eligible to provide letters of support for award applicants.
    APPLICATION PROCESS

    CV or Resume

    Two letters of recommendation, of not more than 500 words, from a professional colleague. To include:
    • How you know the applicant
    • Why you are supporting the applicant for the Rising Star Award
    Current members of the Board of Directors and the Awards Committee are not eligible to provide letters of support for award applicants.

    Applications and supporting materials must be submitted by March 3, 2025. All applications must be complete to be considered. Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results. If a confirmation has not been received by March 22, 2025, it is the applicant's responsibility to contact ADCES at awards@adces.org or call (800) 338-3633 x4810.
