Application Instructions

PURPOSE
Honors a diabetes care and education specialist who has made a special contribution to the field through dedicated, innovative, person-centered clinical care for those with diabetes and related cardiometabolic conditions. The Diabetes Care and Education Specialist will act as an "Ambassador" for ADCES and should be someone who is considered a role model by their peers and colleagues.
AWARD
Applicant will be recognized at the ADCES Annual Conference with a plaque, will receive complimentary registration to the Annual Conference, a $1,000 travel stipend, a $5,000 award, and a travel grant of up to $10,000 to be administered by the ADCES office.

The recipient is asked to do 4-5 virtual/face-to-face educational sessions to present to a Coordinating Body or Local Networking Group in the year immediately following selection. If not a virtual session, he/she is asked to travel to the Coordinating Body or Local Networking Group meeting. The recipient may be asked to also participate in an unpaid, unbranded internal educational/training presentation or panel discussion for the award sponsor.
ELIGIBILTY
Applicant must be, for at least five (5) years of consecutive membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award. Current and past members of the Board of Directors and past recipients are not eligible.

At the time of the application submission, the applicant must be a Certified Diabetes Care and Education Specialist or BC-ADM for at least five (5) years.
CRITERIA
Applicants for the Diabetes Care and Education Specialist of the Year may be nominated by a peer/colleague or by self-nomination.
APPLICATION PROCESS
The applicant must submit:

Three letters of recommendation, of not more than 500 words, from professional colleagues. To include:
  • How you know the applicant
  • Why you are supporting the applicant for the Diabetes Care and Education Specialist of the Year Award
  • How the applicant has made a special contribution to the field of diabetes education through dedicated, innovative, person-centered clinical care for those with diabetes and related cardiometabolic conditions
Current members of the Board of Directors and the Awards Committee are not eligible to provide letters of support for award applicants.

Applications and supporting materials must be submitted by March 3, 2025. All applications must be complete to be considered. Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results.
