Remote Work Survey of Michigan Automotive Component Suppliers
==========> INFORMED CONSENT RELEASE FORM <==========
You are being invited to participate in a research study regarding the Work-From-Home preferences of individuals who are employed by automotive component supplier firms in Michigan. This study is being conducted by Jeffrey K. Baier, a student at Columbia Southern University, as part of the dissertation required for completion of the Doctor of Business Administration degree.
There are no known risks if you decide to participate in this research study, nor will there be any costs incurred by you to do so. The information you provide will be used to determine whether preferences about working from home have changed since remote work measures have been implemented by employers due to the COVID-19 pandemic. This online survey consists of 6 multiple choice questions plus a very brief demographic information request which is optional. Overall, this survey should take less than 3 minutes to complete. Please note that this information collected may not benefit you directly, but the findings derived from this study should be relevant to the automotive component manufacturing industry as a whole, especially as companies seek to establish new Work-From-Home policies moving forward.
This survey is completely anonymous. You will not be asked for your name, employer, or other specific information that could be used to identify you personally. For online survey responses, IP addresses will not be collected. There will be no record of who participated in the study. For any surveys conducted in-person or by telephone the same level of anonymity will also apply. Compiled records may be inspected by Columbia Southern University’s Institutional Review Board. Should the survey data be published, no individual information will be disclosed.
Your participation in this study is voluntary. By completing this survey, you are voluntarily agreeing to participate. Please note that you are free to decline to answer any particular question you do not wish to answer for any reason. Please be aware that once survey answers are submitted they cannot be withdrawn at a later date. Currently, no compensation is being offered for completion of this survey.
If you have any questions about the study, please contact the primary researcher, Jeffrey K. Baier, via phone (248) 460-6712 or e-mail jeffrey.k.baier@gmail.com or Dissertation Chairperson, Dr. Mary Dereshiwsky, by e-mail mary.dereshiwsky@columbiasouthern.edu
