ELECTRONIC PERMISSION SLIP AND LIABILITY WAIVER FOR 8TH GRADE PARTY

The Ladue Middle School Parents Association (“LMSPA”) is sponsoring an 8th Grade Year-End Party for all current Ladue Middle School (“LMS”) students on May 18, 2019 from 7:30-10:00 p.m. at LMS, with a pre-party from 6:30 p.m.-7:30 p.m. on the same date (the “Party”). All students who attend the Party are required to sign a Permission Slip and Liability Waiver as condition to entry into the Party.  You may complete the Permission Slip and Liability Waiver online by completing ALL of the following 10 questions, which includes an electronic signature at the end.

PLEASE READ EACH QUESTION CAREFULLY AND RESPOND TO EVERY QUESTION.  IF YOU SKIP A QUESTION, YOUR PERMISSION SLIP AND LIABILITY WAIVER IS COMPLETELY INVALID, AND YOUR STUDENT WILL NOT BE ALLOWED TO ATTEND THE PARTY.  YOU MUST COMPLETE A SEPARATE FORM FOR EACH 8TH GRADE STUDENT.

4.I understand that my Student’s participation in the Party is entirely voluntary, and is not required by LMS or the LSD.(Required.)
5.I understand that during the Party, my Student may be involved in various age-appropriate activities that may involve some element of risk, which I agree to assume.(Required.)
6.I understand that my Student does not have to participate in any activities offered, and should only participate in activities he/she is comfortable doing.(Required.)
7.I understand that various foods will be offered throughout the Party.  All students with known allergies will be provided a list of the available foods (including the ingredients of those foods) prior to the Party, and allergy-friendly options will be made available, to the extent possible.(Required.)
8.I understand that neither the LMSPA nor the LSD nor their employees, officers, agents or volunteers assumes responsibility for any loss, injury, or damage to your Student or his/her property in connection with his/her participation in the Party.(Required.)
9.I agree to hold the LSD, LMS and/or LMSPA harmless and will indemnify them from any claim or action for personal injury, death, loss of or damage to personal property arising out of your Student’s participation in the Party.(Required.)
