This application consists of 8 sections: Organization & Contact Information, Project Specific Information, Property Site Layout/Infrastructure/Electric Utility Provider Support, Project Timing & Costs, EV Charger User Experience, Operations & Maintenance, Additional Attachments, & Assurances.

Please answer all required questions (depicted by an *) & any additional questions as applicable. Incomplete applications will be automatically rejected.

If needed, please use survey questions #31 – 35 to attach any additional supporting documentation related to this project, including additional explanation & detail for any other survey responses.

This application directly correlates with the Clean Fuel & Charging Infrastructure Program Request for Proposals.

Applications will be accepted on a rolling basis until all funds are allocated.
