2015 USA Ultimate HS Southern Championships Application

Thank you for your interest in attending the 2015 USA Ultimate High School Southern Championships. This online form will save time and allow us to process applications in a more efficient manner.

The Southern Championships will be held in Winston-Salem, NC on May 09-10. High school teams in the south region are eligible to apply for High School Southerns.

Applications must be completed by Friday, March 20, 5pm MT. Notifications will be emailed to invited teams (and alternates) by Friday, March 27. While the application can be completed by any member of the team, an adult (coach, chaperone, teacher sponsor) must be the primary contact.

For teams who are accepted to the tournament, a deposit and preliminary online roster of 12 players ($486.00) is due by Friday, April 10 (5pm MT) or an alternate team may be awarded your bid. This deposit will be applied to the players submitted on your preliminary roster.
