Community Survey

We’re your local Mind, providing mental health support to people in Merthyr Tydfil, Bridgend and Rhondda Cynon Taf.

We are looking at how we can develop and improve our services in future and would love to have the thoughts of everyone in our community - whether you have or previously had challenges with your mental health or supported someone who has.

We would be hugely grateful if you can take up to 5 minutes to answer 4 quick questions, plus some optional ones about you. You can give as little or as much detail as you’d like and skip questions if you prefer.

Question Title

* 1. What are the main issues that affect your mental health or the people you know? (you can list more than one - ideally in order of what has the greatest impact)

Question Title

* 2. What types of mental health related services have you used/currently use - if any? (Select all that apply.)
