Our State wants to hear from family members! What a great opportunity to share your experience with CARE Act.

As we support our families, we may have found that CARE was a vital part in that support, either a care plan, or alternative resources and services offered to our loved one. We may be in the process still, or we may have had some challenges that we want to share.

Disclaimer: CARE Act has been implemented in the cohort 1 counties as of October 1st, 2023. If you are in the cohort 2 counties that are not affected yet till December 1st, 2024, this survey may apply to family members who are supporting their loved one who is located in the cohort 1 county.
