Thank you for providing your feedback about the services you have received from Hamilton Child and Family Supports.
- The purpose of the survey is to learn your opinions about the services provided by us. Our goal is to listen to the voice of those who receive our services and use this feedback to help us provide support to children, youth, and families.
- All responses will be kept CONFIDENTIAL. You will not be identified to any worker and individual responses will not be provided to any workers or recorded on case files. The survey results will only be summarized for the agency in group format (for example, 70% of those surveyed were satisfied with the services they received).
We want to hear from you whether your experience was positive, or you feel there is room for improvement. Your thoughts and opinions are important.
