Thank you for participating in the Virginia Health Care Association-Virginia Center for Assisted Living 2024 Functional Exercise. The following survey will provide our contractor, Incident Management Solutions, with the necessary information to coordinate exercise play during the week of June 24, 2024. We highly recommend completing the survey on a computer device and not via cell phone.

This survey is required for facilties participating in the exercise. Kindly complete no later than Thursday, June 20th, 2024. This survey will take approximately fifteen minutes to complete.

One (1) survey is to be completed per facility/organization. You will need the following information to complete this survey:
  • Facility Command Center or Emergency Operations Center information
  • Exercise control staff contact information
  • Exercise players' contact information
  • Your facility's S.M.A.R.T. exercise objectives
