As clinicians in Niagara we value your input, however know your time is limited. We therefore created this short survey to engage you in two upcoming projects. The NOHT-ESON is developing our Strategic Plan, and would like to hear from you on priorities. As well as the development of a 24/7 Navigation resource. Please take a moment to complete this short survey.

* 3. What do you think the NOHT-ESON health team priorities should be for the next two years?

The 24/7 Navigation project is tasked with implementing and designing opportunities to improve patient navigation within the Niagara region healthcare community. Our work will look to improve access and transitions by designing and implementing navigation services. By March 31, 2023 the project team must implement an initial navigation service that is accessible to the Niagara Ontario Health Team - Équipe Santé Ontario Niagara community. Additionally, this project will be developing a navigation roadmap for the NOHT-ESON to ensure gaps in navigation support for patients, clients, families and caregivers are closed. We are looking for your point of view as clinicians, for the following questions.
