COVID Notification

Richfield Basketball COVID Reporting Form

This form should be completed if a player or coach has tested positive for COVID-19 (even if no symptoms). Please fill out all of the applicable information but leave questions blank if the question does not apply to the situation.

1. For a player who tests positive for COVID:

· Stay home for 5 days from the date of symptoms or date of test, whichever came first. Day 1 is the day after the symptoms or test.

· If your symptoms are improving and you have not had a fever for 24 hours, you can return to activities.

• You must wear a mask at all times (including while competing on the court).

2. For a player who is vaccinated and is exposed to a confirmed COVID case:

· No quarantine is required, but we do recommend you monitor your player for symptoms and get tested.

• You must wear a mask at all times (including while competing on the court).

3. For a player who is unvaccinated and is exposed to a confirmed COVID case:

· Stay home for 5 days from the date of exposure. Day 1 is the day after the exposure.

· If no symptoms develop, the player can return to activities on day 6. A negative test is required to return on day 6.

• You must wear a mask at all times (including while competing on the court).
