The Connecticut Department of Mental Health and Addiction Services (DMHAS) is conducting a survey of tobacco and vape shops across the state. DMHAS would like to get your input to better inform education and information around tobacco and nicotine laws in the state.

This survey will take approximately 10 minutes to answer and is anonymous. Your participation in this survey is voluntary and will assist in developing educational materials to increase your knowledge on state and federal laws on tobacco and nicotine products.
