DC DBH Barrier Survey (Parents and Caregivers)

We would like to express our gratitude for your willingness to participate in our survey focusing on the utilization of behavioral health services in Washington D.C. We are conducting this study on behalf of the Washington D.C. Department of Behavioral Health. You have been chosen to help based on being a parent/guardian or caregiver to a youth ages 5 to 21 years old. All your answers will be kept completely confidential. All information that would let someone identify you or your family will be kept private.
