COVID-19 Survey/Needs Assessment

CLPHA recognizes the need to respond to the unpredictable environment, due to the current COVID-19 pandemic. As the COVID-19 crisis persists, PHA residents continue to be disproportionately at risk for COVID-19. This survey updates information we collected in Winter 2021 and aims to identify and address COVID-19 needs and ongoing priorities for our PHA members, which will help guide CLPHA’s consultation and technical assistance moving forward. It will provide data organized by areas and population of need and will expand our current informal data collection efforts with a neutral and equitable approach. CLPHA staff will analyze the responses and utilize this analysis to drive priorities in seeking additional resources for PHAs and directing staff capacity in areas of PHA need.
Note: You might need to collect information from multiple staff with different specialties. The survey will generally cover the following topics: 
  • COVID-related resident services
  • Costs associated with COVID-19
  • Health and education partnerships
