What is the Gitxsan Community Health & Wellness survey?
The Gitxsan Health Society (GHS) Community Health & Wellness Survey is an opportunity for community members to guide our next 10-year Community Health Plan (CHP) and the Gitxsan Well Being Model. We are asking members about their health and wellbeing, the factors that you believe may be affecting your health; what service and programs gaps you think there are; and the use of health and wellness services. This is an online survey however please contact our office if you would prefer a hardcopy sent to you, of which can be returned in a sealed envelope for an independent review so that your privacy is respected.
What is the purpose of this survey? By collecting information about your health & Wellness, the aim is to: · Provide Gitxsan Health Society with current and detailed information to evaluate existing services and programs and to contribute to new programming for the next 10 years, that is driven by community need; · Provide guidance to help inform any other health strategies and priorities; and · To enhance the Gitxsan Well Being Model.
What are the survey questions about? You will be asked questions about the current state of your health, behaviours thought to be related to your health, your use of health care services and information concerning your family.
Am I required to participate in this survey? The survey is open to all Gitxsan community members who reside both on and off reserve. We encourage all members of your family to complete the survey to ensure there is a wide range of perspectives considered across all age groups.
The survey is voluntary, and you may choose to answer any or all of the questions. In other words, if you do not wish to answer a question just skip to the next one.
Will the information be kept confidential? Yes. Information collected from respondents is kept strictly confidential. We recognize the trust that you place in us to protect your private information. GHS has contracted an independent Indigenous consultancy company to administer the survey and who has contractually agreed to confidential and privacy requirements.
Your personal information will not be shared, and your name and other identifiable information will not be linked to the answers that you provide.
Your name, address, or other identifiable information will only be collected if you choose to enter the prize draw at the completion of the survey. Enter to win a $100 Visa Gift Card.
When does the survey close? All surveys must be received by 31 October 2021.
