Mental Health Awareness & Suicide Prevention Community Assessment

This survey was developed to evaluate gaps in mental health services and the suicide prevention network in Northwest Michigan. It was developed by the Suicide Prevention Action Team, which is part of the Northwest Community Health Innovation Region's (CHIR) Behavioral Health Initiative, and the Northern Michigan United Suicide Prevention Coalition. The results will be used to strengthen supports and services in the region. The results of this survey are anonymous, so please be honest about your experiences.
