The SAFER SUICIDE CARE WORKFORCE SURVEY is a tool to assess staff knowledge, practices, and confidence.
This survey is part of our organizational mission to adopt a system-wide approach to caring for individuals who are at risk for suicide. The results of this survey will help determine our starting point for the implementation of the Suicide Safer Community/Zero Suicide approach, and will be used to identify and address the training needs of our staff.
All responses are anonymous. Please answer honestly, and be thoughtful about your answers even if you do not work directly with suicidal individuals. Unless otherwise indicated, please mark only one answer. The survey will take approximately 10 minutes to complete. By answering this survey, you give your consent to participate; however, you may terminate your participation at any time.
