Thank you for being willing to provide information about walking, biking and rolling to school in communities across Indiana!

The information will be used to develop partnerships and networking, guide training and technical assistance activities, and shape future funding opportunities.

The questions ask about your ‘school/community.’ As you answer, you may respond about one specific school, an overall district, or an entire city or a town. The specific location is less important than capturing the activities, issues and opportunities to be highlighted and addressed moving forward.

Walking and biking to school activities are sometimes part of an official Safe Routes to School program or tied to a specific one-day event, but in this survey, we also want to gather information about informal, everyday examples of active transportation, so no activities are too small!

Will you please share this survey with others in your school/community network, as well?

For more information, please contact Health by Design at info@hbdin.org.

Thank you so much for your time and input! Survey participants will be eligible for incentives including gift cards, complimentary event registrations, and supplies that encourage walking and biking to school!
