Holistic is a people-focused consulting firm that delivers comprehensive solutions for every stage of the employee lifecycle. Our people-first approach prioritizes accurate, self-identified demographic information, which is used to assess employee sentiment, advancement, and opportunity.

Our Demographic Tool will be paired with other data collection methods to gauge the landscape of your company and assess the employee experience across and within demographic groups.

The information you provide Holistic will always remain confidential. All Holistic survey data is owned and secured by Holistic and only used in macro/aggregate for reporting purposes.
1.What gender identity do you most identify with? 

Gender relates to how an individual’s internal sense of being a man, woman, nonbinary, agender,  etc.
2.How would you best describe your race?

Race refers to physical differences that groups and cultures consider socially significant. You can identify with multiple races. (i.e. Caucasian, Black, Asian, etc)
3.How would you best describe your ethnicity?

Ethnicity refers to how you identify culturally based on your national background. You can identify with multiple ethnicities. (Hispanic, Polynesian, Pakistani)
4.How would you best describe your sexual orientation?

Sexual orientation refers to a person's identity in relation to the gender or genders to which they are sexually attracted. (i.e. heterosexual, pansexual, gay, straight, etc)
5.Are you currently serving or have you ever served in the military?
6.Do you identify as having a disability or do you have a physical condition that impacts the way you perform regular tasks required for your role? (All survey responses will remain strictly confidential.)
7.Is there anything else you'd like to share about your disability or health condition?
8.Do you have children or are you a primary caregiver? If so, please list their age/ages.

Why we ask. This allows Holistic to remove employees who leave the organization and to update information for employees who have previously completed the survey. Your individual responses will never be shared with your organization.
