Veterans Health and Wellness Survey

The Washington County Office of Veteran’s Affairs is surveying veterans living in our county. This is a 10 minute survey that asks you questions about your physical and mental health. Our hope is that we hear from as many Washington County veterans as we can to learn more about your care needs. We value your time and opinion. In exchange for your time, you will be entered to win 1 of 3 $100 Visa Cards.

We are working with Parker Philips, Inc. an Erie County, PA based research firm to gather your feedback. Your responses and information will be kept confidential. Your name will not be placed on a mailing list or sold to a third-party for use. 

Please respond by July 31, 2021. Winners will be notified on August 5, 2021.
