Are you (or do you know of) an essential health care worker (of any professional background) who has a story to share about your experience of the COVID-19 pandemic that you think could be helpful for others to hear? If so, please consider completing this nomination form to be considered for a chance to be interviewed by Brandon Stanton, the creator of the street portrait blog, Humans of New York.

IHI is looking for 2 to 3 health care workers who would be willing to record a conversation with Brandon that will be shared with Forum attendees. If you’re interested in nominating yourself or someone else, please submit the following:

*Special consideration will be given to community health workers, nursing home staff, or critical care staff.

Deadline for submissions: Friday, September 10, 2021, by 5:00 PM Eastern Time. Interviewees selected to be part of the panel will be notified by Friday, October 1, 2021.

Interviews will be conducted remotely and IHI will coordinate all filming arrangements. The recording will take place in November and air during the IHI Forum in December. All expenses will be covered by IHI. Length of filming will be approximately one hour, with one prep call in advance.
