Bethlehem Academy Parent Survey for Back to School 2020-2021

As a result of the COVID-19 pandemic Bethlehem Academy, in collaboration with the Archdiocese of St. Paul & Minneapolis, is working to develop plans to address the challenges around this pandemic. We appreciate your input. Thank you for taking the time to complete this survey.
.What is your name?
.What is your email address?(Required.)
.What are your plans this fall for sending your student(s) back to school?
.We are already planning for health and safety in our building, which includes the wearing of masks per the new Executive Order for this age group, but want to know what safety measures would need to be in place for you to be comfortable sending your student(s) to school (select all that apply)
.In your opinion, what is the most important aspect of distance learning? (select all that apply)
.If Bethlehem Academy goes to a hybrid model, would you prefer your student(s):
.If you utilize the bus for transporting your student(s) to school, are you comfortable with the guidelines that would fill a bus at 50%? For example, if the bus had a capacity to seat 70 students, the bus would only allow 35 students to ride at one time. (See the next question if your student(s) ride the BA van.)
.If you utilize the BA van for transporting your student(s) to school, are you comfortable with the guidelines that would fill a bus at 50%? That would mean 4-5 students on the van, with the possibility of doing two runs: one to/from Webster and one to/from Northfield.
.In the event that BA needs to shift to hybrid or distance learning, what is your internet connection?
.6th/7th grade families needing School Age Child Care (ages 3-12): I am an essential worker and my childcare age student(s) will need to attend school daily if BA goes to hybrid or distance learning.
.What other information would you like to communicate that we should include in our return to school planning for this upcoming school year?
