About this survey

For the last two decades, the Knowledge Institute on Child and Youth Mental Health and Addictions (the Knowledge Institute) has been bringing people and knowledge together to strengthen care for infants, children, young people, and caregivers. Since 2018, we've invested in developing quality standards to inform the delivery of high-quality care in Ontario’s child and youth mental health and addictions sector. In 2023, we kicked off work on a new quality standard focusing on Levels of care.

We are excited to now share the Levels of care quality standard with you for feedback! This standard offers guiding principles to develop and implement levels of care models and ensure children, young people and their families can easily find and access the right services that match their particular needs and goals.

Please note that as you complete the survey, your responses on a page will only be saved after you click the “Next” button at the bottom of the page. If you do not complete the survey in one sitting, SurveyMonkey should allow you to leave and return to the survey where you finished, with your previous answers saved on the last page.

This survey will be open from October 17, 2024 to November 22, 2024. The closing time is 11:59PM (EST) on November 22, 2024.

L’ébauche de norme et le sondage sont tous deux disponibles en français.
