This Incident Assessment and Documentation Tool has been developed by MPact to assist LGBTI individuals and groups record inappropriate, stigmatizing, discriminatory, and/or violent treatment during the COVID-19 pandemic. As the situation is quickly evolving in every part of the world, it is essential for our communities to remain vigilant to protect our rights. Information collected from this tool could, upon further investigation and informed consent, be used to report violations to national human rights institutions, regional and global human rights mechanisms, United Nations technical agencies, donors, or other entities. No information submitted on this form will be shared beyond MPact without the express and informed consent of the person who submitted it.
