In order to help ensure the availability of parking for our visitors and patients by enforcing the parking policy, all employees are required to provide vehicle information. It is a requirement and a condition of employment to provide vehicle information and failure to comply shall result in disciplinary action. Employees are required to notify the Director of Safety/Security/Communications of any changes in vehicle type or license plate number. In order to ensure the accuracy of the employee vehicle registration database, employees will be required to resubmit their vehicle(s) license plate numbers at the time of their annual performance review. The Vehicle Registration Information Form must be submitted at this time even if there has not been a change in license plate information.
