Thank you for taking the time to complete this survey! This Community Health Needs Assessment (CHNA) is being conducted as a collaborative effort between Holy Cross Medical Center (HCMC) and Ovation Healthcare. Your responses will help inform the top health priorities in your community.

This survey will ask you to complete the following:
  • Rank the health needs of your community
  • Provide your opinion of the last CHNA performed in 2022
  • Complete questions related to access to services in your community
