The information gathered from this critical annual survey is used in conjunction with our overall data analysis, culminating in HCA's annual report on the financial condition of the home care industry in New York state. Your responses play a key role in the development of our state and federal advocacy plans.

This year, respondents will submit only one survey for their agency that will gather information from each of their entity types (CHHA, LHCSA, Hospice, MLTC/PACE, and/or CDPAP/FI).

To complete this survey, you will need access to the following report submissions:
  • CHHAs, LHCSAs and FIs - 2023 Medicaid Home Care Cost Report
  • Hospices - 2023 Cost and Utilization Report
  • MLTC and PACE Plans - 4th Quarter, 2023 MMCORs

We recommend the following steps to facilitate your survey submission:
  1. Download the PDF version of the 2024 - 2025 HCA State of the Industry Survey here.
  2. Review all questions to see if you need to delegate certain questions to appropriate staff on your team.
  3. Once questions are complete, return to this online version of the survey to submit your answers.
  4. Please note that each question requires an answer before moving to the next page.
Responses are due by close of business (COB) on Thursday, October 10, 2024.

All responses are confidential and will only be reported in the aggregate.
