Group Class Registration Form and Dance Interest Survey-Future Classes 2021

This is a Registration and Interest Survey for future classes when we can return to Ballroom and Latin Group Classes, taught by Ralph Hunn, Professional Dance Instructor, owner of Affordable Dancing, in Myrtle Beach, SC.  To schedule and plan future classes to be taught will depend upon the interest of those who complete this survey, are in his database and contact list. Also holding classes depends upon the number of participants (as there is a minimum number for a good class),  availability of his schedule and those of interested potential participants. If you received this survey by e-mail, it is because there has been expressed interest in participating in his group classes or you have previously attended one he taught.
By completing this survey, you will provide Ralph the ability to create a great group class which will be designed to meet the needs of those interested in taking his group classes.  Classes are open to singles and couples.
Your contact information and survey answers is confidential and will only be provided Ralph and/or his assigned associate who has been selected to work with him for planning purposes.

When group classes are held, they  are prepaid at a cost of $60.00 per person (a couple $120) for the six week time period. Prepayment would be appreciated one week prior to the first class to determine how many will be attending. You also may pay the full amount at the first class. Payment is to be made by check (payable to Ralph Hunn) or cash. Credit cards are not accepted.
If there is not enough interest, and prepayment has been made, as the class may not have enough participants, (six participants are needed for a group), then refunds will be made.
Also, I understand that refunds are not provided after the first class unless for unexpected, documented illness or emergency reasons.
