SUPPLEMENTAL APPLICATION QUESTIONNAIRE

*The GRAPHIC DESIGNER I examination is being given on a Servicewide, Open basis. The examination will consist solely of this Supplemental Application Questionnaire. This questionnaire is your entire examination and is designed to elicit a range of specific responses and information regarding each candidate's knowledge, abilities, and experience to effectively perform the duties relative to the classification. Candidates are responsible for reading all of the information provided prior to completing this questionnaire. Responses will be evaluated using a predetermined rating criteria. In order to obtain a position on the eligibility list, a minimum score of 70% must be attained. Please answer questions completely, since incomplete responses and omitted information cannot be considered and/or assumed. Resumes, letters of reference, and other materials will not be evaluated or considered as responses to items in the Supplemental Applicaiton Questionnaire.

(NOTE: Failure to meet the minimum qualifications and/or to complete this questionnaire accurately will result in elimination from this examination.) Candidates who fail to follow the instructions and/or who solicit input or assistance from others to complete this questionnaire will be eliminated from the examination.)

This examination requires candidates to respond to 4 supplemental questions. The examination answers cannot be saved for completion at a later time; therefore, the examination MUST be completed in one session. Please ensure you have at least 45-60 minutes to complete this process before beginning.

Examinations are processed on a quarterly basis. The final submission date is on the last day of the quarterly submission by 11:59 pm. Pacific Standard Time (PST). The Supplemental Application Questionnaire must be completed and submitted by the time and date mentioned above to be processed. To submit any work completed hit the 'CONFIRM RESULTS' button before the quarterly submission date or your examination will not be considered until the next administration.

Candidates will be notified of their scores approximately 4-6 weeks after each cut-off date.

The California civil service selection system is merit-based and eligibility for appointment is established through a formal examination process.  The Graphic Designer I examination consists of a Supplemental Application Questionnaire used to evaluate your education, training, and experience.  The eligible list resulting from this examination process will be used by all state departments to fill their existing positions.

This Supplemental Application Questionnaire for 100% of the weight of your examination for this classification.  Therefore, please be sure to follow the instructions carefully.

Before a final score is determined, your responses to exam questions will be verified.  An exams manager or personnel staff member may contact the individuals or educational institutions you have provided to confirm job dates, experience, duties, achievements, and/or possession of knowledge, skills, and abilities.  Failure to provide adequate employment and/or education information may result in a low score or disqualification from this Examination. 

If it is determined at any time that you have made any false or inaccurate representations in any of the information you have provided on this Examination, you may be disqualified from this process, removed from the certification list(s), suffer a loss of State employment, and/or suffer a loss of the right to compete in any future State of California hiring processes.  You are solely responsible for the accuracy of the responses provided.
