This Olmsted County Community Health Needs Assessment (CHNA) prioritization survey helps identify the most pressing health issues in our community. It is part of the CHNA work led by Olmsted County Public Health Services, Olmsted Medical Center, and Mayo Clinic to understand and improve our community health. The Olmsted County CHNA is a collaborative effort that began in 2012. Information from our most recent CHNA will be available in October 2022.

 

This survey asks you to think about various health issues and how they impact our community. We will ask you if we need to address the health issues soon. No data will be shown, we just want your opinions.

 

The survey should take about 15 minutes. Please note that you need to complete the survey in one sitting. You cannot exit out of the survey and resume it later. Thank you!
