This survey is to assist in a training needs analysis for the Agency at large. All responses are completely anonymous and will not be collected or used for performance of any employee.  Please be as honest as possible when responding, we don't know what is needed without your input.  Upon completion of the survey, you or a co-worker may be asked to participate in a focus group to qualify the survey data.  Please note that during this process again, your responses will not be linked to any person.  This will only be used to further define future training needs of our Agency employees.
