UC Heart Disease Prevention: Community Survey

Heart Disease is the leading cause of death in Ulster County and the nation. The Ulster County Department of Health, and its community partners, have formed a Heart Disease Prevention Team tasked with assessing resident's awareness of heart disease risk and prevention factors. We will combine the findings, along with other data and information, to help develop a Strategic Plan of Action designed to bring heart disease related fatalities down over time.
