Program Accessibility Survey

This survey is designed to better understand the landscape and needs of afterschool programs, particularly as they relate to serving youth with disabilities. By gathering feedback, we hope to increase our understanding of how programs foster inclusive practices and environments that ensure all students, regardless of background or ability, have equitable opportunities in afterschool programs.

The information collected will help: identify strengths and areas for improvement in supporting youth with disabilities in afterschool and out-of-school time (OST) programs; guide the development of professional development and training opportunities; and inform strategies to enhance program accessibility, inclusivity, and enrichment opportunities for all youth.

This survey will NOT be used to evaluate individual programs or staff performance and will not be shared in a way that identifies specific individuals or programs.
