Connected Minds Self-Identification Form (Affiliate Members) |
Basic Information
To better understand the diversity of the Connected Minds Affiliate Member program and create a more equitable and inclusive funding program, we are inviting affiliate members to provide voluntary self-identification data. The survey contains 13 questions and will take approximately 5 minutes to complete.
Even if you are choosing not to self-identify or have chosen not to self-identify in the past we invite you to complete the self-identification form by selecting the "I prefer not to answer" option available throughout the survey.
Please note that the self-identification information can only be accessed by the Connected Minds Program Director (Caitlin Mullin), Operations Manager (Tara Tharani), and EDI and Community Outreach Specialist (Andrew Castillo).
No identities will be linked to any names on the affiliate member application, this is done to protect the identities of individuals. Self-identification information will be used to create a templated report that will report disaggregated proportions of the four federal designated groups (women, Indigenous people of Canada, Persons with disabilities, members of racialized groups), 2SLGBTQIA+, and French linguistic minority status. In the situation where less than 10 people self-identify, specific percentages will not be provided. These practices have been enacted to maintain the anonymity and confidentiality of respondents while allowing Connected Minds to receive the data they need to build a more diverse, equitable, and inclusive environment as part of our annual reporting cycle. Self-identification information will be password protected and kept on a secure server only accessed by the Connected Minds staff members mentioned above. Information will remain in the server until 2037 as mandated by CFREF.
If you have any questions, please feel free to email Andrew Castillo (drewcast@yorku.ca), EDI and Community Outreach Specialist for Connected Minds.
