The results of this survey will be presented at DC PACT's next meeting on Thursday, October 12th and will form the basis for building consensus on DC PACT's recommendations for an effective Medicaid section 1115 waiver.

Should DC's AE for Team-Based Care be Managed Care Organizations (MCOs), a single other entity (such as an ACH), or multiple other entities (such as health systems or care networks)? The state 1115 waiver crosswalk we presented at the 9/27 PACT meeting broke down each waiver into 4 sections: 1) Investments in Community Based Care, 2) Health Equity Action Element, 3) Health Related Social Need (HRSN) Services, and 4) Upstream Infrastructure/Social Conditions. In the following questions, we've broken each of these sections down into 4-5 general elements.
