Demographic Information
The purpose of the following survey is to gather some factual data about you and your experiences. All responses are confidential unless you wish to share your identity for this research. Any information disclosed will be with your permission. While there may be publications and presentations as a result of this study, your name will not be used unless you choose to reveal your identity in the questions below. If you participate in this study, we will quote directly without using names. Names of people and places will be changed in transcripts and other data by the research team as the data is collected. All information will be kept in a locked cabinet in a locked office at the university.
Upon completing this survey, you may be selected to participate in 2 interviews for a total of $100, paid upon completion of both interviews.
Your participation is completely voluntary. You are free not to participate, or to stop participating at any time, for any reason without negative consequences. In the event that you do withdraw from this study, the information you have already provided will be kept in a confidential manner. If you wish to withdraw from the study, please contact Amanda Moon.
