Mullberry Patient Feedback Survey

We have been made aware by some patients that walk-in appointments do not work for them. We want to understand what will work best for patients who need to access appointments for urgent health needs. We are using a system where we assess the need based upon answers to some questions about your health in other practices and we know some Mulberry patients are requesting we implement it here. If we did, you can ask for appointments over the phone or online by explaining what the problem is, this is called triage.
1.Our triage model will allow patients to call or go online and self-service. We will not allow people to walk in to make an appointment.

Do you believe this provides enough ways to contact the practice?
2.The online system is available in English, Romanian, Punjabi, Hindi, Urdu, Polish, Bengali, Russian, Lithuanian and Albanian. We have also asked for translations to Arabic, Pashto, Dari and Kurdish.

Do you feel like their will be an issue with trying to answer questions on the phone or online?
3.Do you feel you will be able to access services?
4.Would you prefer to walk-in and wait or schedule appointments at a set time and date?
5.Will you be able to receive texts from us about your appointment times on a mobile phone whichever way you contact us?
6.Please provide any other comments you have
