Every Birth Network Community Survey

Introduction

Local service providers want to better understand health outcomes to improve local services to families. Help us better understand these factors that affect your health by completing this survey about your community, social life, stress, and everyday life. We believe that most people will complete the survey in about 15 minutes.

If you had a baby in the past 2 years or are currently pregnant, and live in Allen County, Indiana, we need you to complete this survey. You have to be between 18 to 49 years old to complete the survey.

It is important that you answer all of the questions.

There is no right or wrong answer, and we only want to know your answers, not what you think your doctors, family, or friends might believe.

Upon completion of the survey, you will have a chance to enter a drawing to win $100. Up to five people may win.
