CAC2 Survivorship Toolkit - Insurance and Financial Health

Thank you for your interest in this important resource designed to help childhood cancer survivors and their families. We are hoping to collect resources and information that you have found to be helpful when supporting your own situation, so that other families can find these resources and benefit from them . Many of the resources you may have found/used may not be childhood cancer specific, and that is OK. In fact, we are expecting it.
 
We are looking for what will provide support to families dealing with the insurance challenges or financial toxicity that exist in survivorship. Please review these categories below. Provide any organizations/agencies, websites, or resources you have found helpful, or believe would be helpful to families.  We are looking for information speaking to the parents, and specifically to the survivor/student. YOU DO NOT NEED TO ANSWER EACH SECTION, please contribute to any of these areas as you can. When completed, scroll to the bottom and click on DONE.

1.Information related to applying for or managing Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI).
2.Financial Toxicity - information to explain what it is, and how to manage finances while juggling bills from treatment plus any potential additional bills resulting from survivorship issues.
3.Health insurance for survivors when they turn 26 years old.
4.Medicare/Medicaid availability for survivors
5.Legal support/Conservatorship in healthcare, or development of special needs trust
6.Dental/Vision/Hearing Aid insurance coverage for cancer-related health issues
7.Mental health support/benefits for psychosocial support services or to manage depression/anxiety for survivors and their families
8.Life insurance information for childhood cancer survivors
9.Preparations by parents/siblings for dependent survivors when parents no longer can care for them because of poor health/death
