Needs Assessment for People Living with Brain Injuries, Caregivers, and Family Members |
This survey will take approximately 10 minutes of your time.
Please take this opportunity to provide important information about your experience with traumatic brain injury (TBI), your needs, and your opinions about the successes and challenges related to the resources and services that you receive(d). Information will be used to support a state plan for TBI with the goal of improving quality of life for this under-served population.
The survey is voluntary and anonymous, and you will not be individually identified. Staff members of the TBI State Partnership Program and the Arkansas Trauma Rehabilitation Program will compile and summarize the results, which will be available for public review at a later date.
