Assisted Living Feedback Forum Registration

Welcome

Continuing its aim to improve the quality of care and quality of life for long-term care residents across the state, the Ohio Department of Aging will be hosting a series of in-person Assisted Living Feedback Forums in August and September in multiple locations.

We want to hear from you! By participating in a Feedback Forum, you can share your personal experiences, make your voice heard and help us make future enhancements to Ohio’s Nursing Home Quality Navigator by adding assisted living metrics.

Question Title

* Your attendance at a meeting constitutes your consent to be photographed, filmed, and recorded, and to the release, publication, exhibition, reproduction of any and all media for use on websites, in social media, news and publications. By attending a meeting, you waive and release any claims you may have related to the use of the recorded media. While any personally identifying information provided would be protected, testimony provided would be subject to a public record subject to release on request by a member of the public.
