EMR Grant Application form must be completed and submitted online by Sunday, July 14th

Reminder to LFM physicians: EMR grants B and C are included in the longitudinal family medicine payment model. There is no need for LFM physicians to apply for these grants.

Please read the following instructions carefully.

Once you open the online application, you must complete it in one session (approximately 10 to 30 minutes). You will not be able to save your changes and return to the form at another time.


1. Deadline - please submit ASAP
The EMR Grant Application form must be completed and submitted online by 11:59 p.m., Sunday, July 14, 2024 to be considered for the EMR Participation Grant and/or EMR Utilization Grant payment. Late applications will only be accepted if previously approved by the Department of Health and Wellness and Doctors Nova Scotia.

2. Eligibility Criteria
To be eligible for the EMR Participation Grant, you must have:
• used an office-based EMR system that meets the Canada Health Infoway definition
• implemented the EMR by March 31, 2024
• invested at least four hours in EMR participation events and/or activities during the 12-month period from April 1, 2023, to March 31, 2024, as described in the application form

To be eligible for the EMR Utilization Grant, you must have:
• used an office-based EMR system that meets the Canada Health Infoway definition
• implemented the EMR by March 31, 2024
• billed (or shadow-billed) a minimum of $30,000 in MSI billings only (excluding hospital-based activities, non-insured services and Workers’ Compensation Board payments) within an EMR in the 12 months leading up to March 31, 2024

3. Your application should be completed based on how you and your office staff use the EMR, not on the system capabilities.

4. Each question must be answered. Please consult with your office administrator or office manager to ensure accurate responses.

5. Each response will be assigned a score. The total score will be used to calculate individual payment.

6. Your MSI Provider Number is required. If unknown, ask your billing clerk.

7. For Part B, Participation Grant Application Form, you may be contacted for verification of your participation in peer educational and CME activities. Please ensure you can provide proof (such as dates and names of courses, journals and so on).

8. For Part C, Utilization Grant Application Form, you could be contacted for verification of the accuracy of your responses. This is to ensure that the limited available funding is appropriately and fairly allocated to each EMR user.

9. The Department of Health and Wellness expects the payments will be made in September.
