Part-Time Bilingual (Spanish/English) Hybrid Community Health Worker
The Improving Community Outcome and Maternal and Child Health Initiative is intended to improve birth outcomes, reduce infant mortality, and improve the health status of children ages birth to five. To address the complex challenges of improving the health of women of childbearing age and children ages birth to five, Durham County stakeholders have chosen the following three evidence-based strategies:
1. Preconception and Interconception Health
2. 10 Successful Steps for Breastfeeding, with a specific focus on Step 3 and Step 10
3. Family Connects (newborn home visiting program)
We are seeking to hire a Community Health Worker, a contracted position to assist the ICO4MCH network by continuing efforts and leveraging partnerships that Durham County has successfully implemented to reduce health inequities in access to services by providing information, services, resources, and supplies to women and men of reproductive age.
