Post-Training / Clinic WIC Director Survey

Thank you for taking the time to complete this survey.  If you have had a new Clerk or a new CPA attend our Hybrid WIC Training Center in the last few months or so, then we would like you to complete this survey.   Please complete a separate survey for each staff who attended.  This survey should be completed after you have had a chance to observe your new staff working for at least a couple of weeks after their training.
