NYC DOHMH Survey |
Informed Consent and Eligibility
We need your participation in a study conducted by the New York City Department of Health to help us learn more about healthcare practices among providers in New York City.
First you will need to complete a few screening items to see if you are eligible for the survey. If you are eligible, you will be directed to a survey that will take about 5 to 10 minutes to complete. You will be asked to answer a few questions about yourself and your practice.
If you are one of the first 355 respondents who complete the survey, you will be awarded a $50 Amazon gift card for your time.
Your participation in this research study is voluntary, and you can stop at any time. The answers you give will be kept confidential and will not be linked to your name.
