Survey Information

There are a disproportionate number of ethnically diverse people accessing reablement* and intermediate care** compared with White British people.

*Reablement/ Home first is a short-term service which supports people by providing care in their home to maximise and maintain independence. It helps people to retain or regain their skills and confidence so they can learn to manage again after a period of illness.

** Intermediate Care Units provide support in a care home setting for a short time to help patients recover and increase their independence, with access to support such as physio therapy and occupational health.
Care is provided at:
Moorland Grange in Huddersfield
Ings Grove in Mirfield
Brackenbed View (Oak Unit) in Halifax

We are aiming to understand the reasons why fewer people from ethnically diverse backgrounds are accessing care after hospital discharge.
This survey is for people over the age of 18 who live in Kirklees or Calderdale.
