Thank you for taking the time to participate in this important research study on the factors that are involved in a decision-making process. The survey will take approximately 10 minutes to complete. By starting this short survey, you are consenting to participate in this study. At any point, you may stop the survey as the responses are only collected at the end after submission.

Instructions:
Simply choose the best selection among the choices. There is no scoring of right or wrong answers. You cannot go back to reread any prompts while answering questions. Note that this is not a typical memory recall survey. This survey is intentionally designed to ask questions without prior information.
