DAISY Award for Extraordinary Nursing Students Nomination Form
Chamberlain University is proud to partner with the DAISY Foundation to present the DAISY Award for Extraordinary Nursing Students to outstanding students whose actions demonstrate caring, advocating and/or leading.
We are seeking nominations from students, classmates, faculty, colleagues, patients, patient's family, preceptors and/or clinical partners.
Use this form to tell us how you have been impacted by the student you chose to nominate. First, you will be asked to provide some information about the student and yourself. Next, you will be asked to rate the student on five topic areas and provide clarifying details that help explain why you chose particular responses. Lastly, make your nomination stand out by providing a detailed example of a specific interaction that was particularly impactful. All in all, this form may take 15-20 minutes to fill out. Only complete submissions with descriptive supporting statements will be considered.
All Chamberlain University, College of Nursing full-time, part-time and degree-seeking students either on-campus or online are eligible to receive the award.
Nominations will be reviewed by the DAISY Awards Committee. Awards are distributed at least one time per year, but you can nominate a student any time of the year.
