The information schools provide in this form will be used by Youthworks Consulting to communicate how to use the Cybersurvey and to share the results. In the rare event that there is safeguarding concern due to a young person's response, we will contact you via the email and phone number you provide. This form is on an encrypted and GDPR compliant survey system. Please be assured that no school or student names will be shown within the survey results. We do not share data with any third parties.
